Video Permission Forms

Participant-recorded video activities will require permission from the site and the families of the young children. The following forms will be helpful and should be customized to indicate your learning institution:

Best Practices

Arrange for permission at the beginning of your course term and keep copies of the completed forms in addition to submitting them to your course instructor.

Conduct your video recordings early in case you may need to re-film or if you require additional footage before your assignment is due.

Offer to share the recordings of your children with their families, these videos make good artifacts for observation and assessment needs.
