BHSS Curriculum Feedback Form
You can access the feedback form here: BHSS Curriculum Feedback Form (opens in a new tab). Complete the provided form fields; required fields are marked “must provide value” in red font. Select which BHSS resource you are providing feedback on, then complete the customized fields that appear.
The BHSS Project Team reviews feedback submissions at regular intervals. If you believe your feedback needs urgent review by the team outside of their regular review cycle, please select “Yes” on the final question of the feedback form.
Once you have filled in the necessary information, select “Submit” at the bottom of the page. A confirmation page will appear.
